About Us

Wallsend Primary Care Network (PCN) started in 2019. We are a group of 3 Practices, working together to improve the health of over 40,000 patients in Wallsend. We are based in Wallsend Health Centre – some of our teams are based here, and we also offer some appointments from here (these are usually booked by your practice). We also offer appointments from within your practice. 

Having a PCN allows us to deliver care which is personalised to the needs of the patients of Wallsend, both reacting when you are an unwell, and trying to prevent illness. To do this, we run services and work closely with our partners in the local community and NHS.

Our Services

Social Prescribing

There are Social Prescribing Link Workers in all of our practices, and a Health and Wellbeing Coach based at Family Gateway. Our aim is to support you to access local services based on what matters to you, for example getting involved in social or community activities, increasing fitness levels, improving mental or emotional health, or finding support with practical issues like housing or finances.

Pharmacy Team

The Pharmacy Team is made up of Pharmacists and Pharmacy Technicians, who mainly work within the practices. They have multiple roles, including reviewing medications, improving medicine safety and managing medication for Long Term Conditions, for example High Blood Pressure. The Pharmacy Team also work closely with the practice admin teams, for example updating medications if you have been discharged from hospital or helping with prescription requests. If required, you may be able to book an appointment directly via your practice, or you may be referred by a clinician.

Physiotherapy Team

First Contact Physiotherapists specialise in the diagnosis and management of musculoskeletal conditions. They mainly work from the Wallsend PCN Hub but also offer some appointments from the practices. Please ask your practice reception team for an appointment

Paramedic Home Visiting Service

A same-day home visiting service for short term illnesses in housebound patients, offered in partnership with North East Ambulance Service. Your practice will triage any requests

GP Assistants offer clinical and administrative support to GPs. This allows practice teams to focus on direct patient care

Medication Review Service

Hosted by TyneHealth, we have dedicated pharmacists and pharmacy technicians who review your medication annually, making sure all monitoring and reviews are completed and that the medication is safe to continue. This is often done remotely via the notes, rather than during an appointment. Historically, GPs spent time doing this work, and by setting up the Medication Review Service, we have been able to offer more appointments to patients who want to see their GP.

COVID Vaccination

Wallsend PCN helps with the logistics of COVID vaccination, for example coordinating vaccine delivery. By working at scale, this saves time for practices to focus on direct patient care.

Extended Access

We offer appointments at various times throughout the day, including weekends and evenings, at the Battle Hill Health Centre. This service is run by TyneHealth and there is more information here. Please contact your practice to book into these appointments.
